1. Night 01: Cursed City

Tokyo is full of curses. Misa lost the memory of a year and wandered alone in the city. Tagami meets Misa as he searches for the increasing number of disappearances in the city. And in the city of curses, Hiromi, an entertainer with an evil eye who curses people just by looking at them, tried to use his power to reach his ambitions.

2. Night 02: Ritual

January 13th, 2004

Mitchie, a runaway girl, has gone missing and is rumored to have been murdered. Tagami, who is investigating the case, learns of a group that is obsessed with strange rituals. Since Mitchie went missing, strange phenomena have been happening to Yuka. Misa helps her as she walks away as if being pulled by an invisible hand. Then, a man suddenly appears behind her and hits her in the back of the head, causing her to lose consciousness.

3. Night 03: Red Room

January 20th, 2004

The manager Ryouko becomes interested in Misa, who arrives at the SM club "Red Room" chasing the actress Linda, worker at the house. Ryouko tries to look into Misa's past, as Misa claims to be a witch, but she cannot, as Misa has lost her memories of the past year. Meanwhile, Tagami finds a document with Misa's photo attached to it in a file on the desk. However, the name in the identification section reads "Tomoda Chisa".

4. Night 04: Flower of the Evil

January 27th, 2004

During his investigation, Tagami notices that a talent named Hiromi Yamanaka is somehow connected to the mysterious incident and begins his investigation. Meanwhile, Hiromi, at talent with the evil eye, focuses her attention on the president of her agency and the popular model Hikaru to achieve her own ambitions.

5. Night 05: Castle of the Curse

February 3rd, 2004

A female teacher at Minamibuchi High School left traces of a curse before committing suicide. After being taught the meaning of the sigil during a mass, Eri also hears rumors related to the school. "There’s a teacher who disappeared a few years ago and was never found." Intrigued by this rumor, Eri heads towards a normal house with a sign reading "Omura Clinic" in order to uncover the truth behind the rumor.

6. Night 06: Monster

February 10th, 2004

An incident occurred in which someone cut an eye with a sharp blade. This incident was part of a series of random stabbings that had occurred three months earlier, and the perpetrator at the time, Sakiko Nukata, had been arrested. However, she escaped and began repeating her crimes. What Sakiko sought was a powerful eye, the eye of Misa. Meanwhile, Tagami was investigating the mystery of the missing "Tomoda Chisa"...

7. Night 07: Compound Eye

February 17th, 2004

Kaoru, feeling the abnormality in his eyes due to the curse cast by Yuina, begins to grow confused. Eri, determined to help Kaoru, searches for Misa. Meanwhile, Hiromi, who has caught the attention of the House of Representatives member Sakuragi, is given a luxury apartment. In the program where she works as a reporter, she successfully reports a major scoop, and it seems like she's progressing smoothly toward a promotion to main anchor.

8. Night 08: Ghost Body

February 24th, 2004

Yuina, frightened by the effects of the curse she placed on Kaoru, burns the cursed doll and notebook in an incinerator. In contrast to Kaoru, whose curse has been lifted, many strange incidents begin to occur around Yuina.

9. Night 09: Eye Seal

March 2nd, 2004

In the news program where Hiromi serves as the anchor, a topic of discussion emerges about an abnormal visual person who can see something that ordinary people cannot. Meanwhile, in the red room, Ryoko begins to speak about her past.

10. Night 10: Flow

March 9th, 2004

Seeking the lost memories of the past year, Misa continues to walk aimlessly through the streets at night, eventually finding herself in a cemetery in the dead of night. As she traces her old memories, she recalls that a year ago, her memory had been cut off at this very spot. What Misa begins to see as she retraces her memories is...

11. Night 11: Witch Hunt

March 16th, 2004

A strange disease, named the Basilisk Syndrome, in which a third eye appears on the forehead, began to spread primarily in the metropolitan area. Patients were being swiftly restrained one after another due to reports. The streets were filled with fear, suspicion, and hatred. Amidst all this, Tagami was pursuing the mystery left behind by the journalist, Hagiwara.

12. Night 12: Witch's Hammer

March 23rd, 2004

The television news was broadcasting about the first carrier of the Basilisk Syndrome. On the giant street TV, Misa’s face appeared, surrounded by the whispers of the rioting mob. In the center of the pentagram, Misa raised Asamei to her chest and began chanting a spell. At that moment, the barrier was broken, and from within the black shadows, Hiromi appeared.

13. Night 13: Hell's Gate

March 30th, 2004

To prevent the Hell Gate from opening, Misa performs a ritual to bind the witches' powers. Among the five "witches," what Misa sees with her eyes is her own figure standing at the entrance of the Hell Gate. "Kuroi Misa will curse and kill Tokyo..." What is the true meaning behind these words left by Hagiwara, and the true form of Kuroi Misa...?
